Your first visit to Watertown Dental Care establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and presenting treatment options. Occasionally, treatment can be done at this first appointment. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at your consultation:

• Any current x-rays.
• Patient history and welcome forms (available below)
• A list of medications you are currently taking.*
• Your insurance identification card if you have dental insurance
• We recommend a parent or guardian accompany all patients under the age of 18 at the initial visit.

*Please notify the office if you have a medical condition that requires pre-medication prior to dental treatment.

X-Rays
If you have had recent x-rays taken (within 6-months) at another office, you may request that your previous dentist forward them to our office. If there is not sufficient time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our office.
